Psychiatrist, psychologist, clinical social worker or counsellor? A plain-English guide to who does what in Alberta, what is covered and how to choose.
A psychiatrist is a medical doctor who can diagnose mental illness and prescribe medication. A psychologist is a regulated professional trained in psychological assessment and therapy. A Registered Clinical Social Worker (RCSW) is a regulated social worker with advanced clinical training in counselling and psychotherapy. “Therapist” and “counsellor” are general words for anyone who provides talk therapy, so the credentials behind them matter. For most everyday concerns, such as anxiety, stress, relationship problems or grief, any regulated professional trained in therapy can help. The best choice depends on what you need, how you will pay and who you feel comfortable with.

- Family doctor: often the first stop. Can screen for common conditions, prescribe, rule out physical causes and refer you on.
What does a psychiatrist do?
Psychiatrists are physicians, so their training focuses on the medical side of mental health. They are most helpful when:
- Medication may be part of treatment, or current medication is not working
- Symptoms are severe, such as psychosis, bipolar disorder or severe depression
- There are complex medical issues alongside mental health concerns
Many psychiatrists see people for a consultation and medication review and then return care to a family doctor. Weekly talk therapy with a psychiatrist is less common. In Alberta you usually need a referral from a physician.
What does a psychologist do?
Psychologists are trained in human behaviour, psychological testing and evidence-based therapy. In Alberta, psychologists are registered with the College of Alberta Psychologists. They are often the right choice when you need formal psychological testing, for example a full diagnostic assessment for a learning disability, or psycho-educational testing for school accommodations.
What does a clinical social worker do?
Social workers are trained to look at the whole person: your mental health, relationships, family, culture, work and community. A Registered Clinical Social Worker has gone further, completing an MSW, supervised clinical practice and the clinical exam to join the ACSW clinical registry. RCSWs provide individual, couples and family therapy using approaches such as CBT, EMDR and the Gottman Method. They also complete many types of assessment. At Disha, for example, our RCSW provides mental health assessments, ADHD screening and capacity assessments as a designated capacity assessor.
What about “therapist” and “counsellor”?
These words describe a role, not a specific credential. A therapist might be a psychologist, a clinical social worker, a counselling therapist, a marriage and family therapist or someone with much less training. That is why the first question to ask is not “are you a therapist?” but “what is your registration and training?”
Who is covered by insurance in Alberta?
- Alberta Health Care Insurance Plan (AHCIP): covers physicians, including psychiatrists, with a referral. It does not cover private psychologists, social workers or counsellors.
- Extended health benefits: most workplace and private plans cover some sessions with a psychologist, and many also cover Registered Social Workers or clinical social workers. Each plan is different.

How to choose the right professional
Start with what you need:
- You want to talk through anxiety, stress, low mood, grief, trauma or a relationship issue: a psychologist or clinical social worker trained in therapy is a good fit.
- You think medication might help: start with your family doctor, who can prescribe or refer you to a psychiatrist. Therapy and medication often work well together.
- You need a formal diagnosis or testing for school, work or legal reasons: ask whether the professional provides that specific type of assessment and what the report will include.
- You want support for your relationship or family: look for someone trained in couples counselling or family therapy.
Then think about fit. Research on therapy consistently finds that the working relationship between client and therapist is one of the strongest predictors of progress. Language, culture and comfort all matter. If it would help to speak in Hindi, Nepali, Punjabi or Urdu, or to work with someone who understands South Asian family life, it is reasonable to look for that.

Is a therapist the same as a psychologist?
Not always. A psychologist is a regulated professional. “Therapist” is a general term that can describe a psychologist, a clinical social worker, a counsellor or others. Ask about registration.
Can a psychologist or social worker prescribe medication?
No. In Alberta only physicians, including family doctors and psychiatrists, and some nurse practitioners can prescribe. Therapists often work alongside your doctor.
Is a clinical social worker as qualified to provide therapy as a psychologist?
