When there are concerns about safety, such as self-harm, harm to others or family violence, a clinical risk assessment helps identify risk and protective factors and creates a clear plan to keep people safe.

Quick answer
A clinical risk assessment is a structured evaluation of the likelihood of harm, such as self-harm, suicide, violence or family violence, based on interviews, history, current stressors and protective factors. A trained clinician summarizes risk and protective factors and recommends a safety plan, treatment and supports to reduce risk.
If someone is in immediate danger, call 911. For suicide or crisis support at any time, call or text 9-8-8.
